I know that if you search “chiropractic” online, or scroll through TikTok, you might eventually encounter scary headlines linking neck adjustments to strokes or arterial dissections. It is completely normal to feel concerned when you see this. In fact, at Revive Active Chiropractic, I welcome these questions because it gives us a chance to look at what the highest quality science actually says, rather than relying on fear-based anecdotes.
I want to walk you through the real data so you can feel confident in your care.
Let’s address the fear directly: Can a neck adjustment cause a stroke? For years, case reports suggested a link. However, modern research has dug much deeper to distinguish between bad timing and actual cause.
The specific condition usually discussed is a Vertebral Artery Dissection (VAD). This is a rare tear in the artery lining that can lead to a stroke. The important thing to know is that these dissections often cause symptoms, specifically neck pain and severe headaches, before the stroke occurs.
To understand the risk, we have to look at a concept researchers call “Protopathic Bias” (or reverse causality).
Here is the scenario: A patient develops a spontaneous arterial tear (dissection). This is a rare event that can happen from everyday activities. The early symptoms of this tear are often sudden neck pain and a “new, different” headache.
Naturally, the patient seeks neck pain relief. They might go to a chiropractor, or they might go to their Primary Care Physician (PCP). If the stroke progresses a few days later, the last doctor they saw often gets blamed.
Think of it this way: If a person is having early signs of a heart attack and walks into a cardiologist’s office, and then has the heart attack, we don’t blame the cardiologist. We understand the patient was there because of the problem, not that the doctor caused it. The data now proves this is exactly what happens with neck pain and stroke.
When researchers looked at huge populations to test this “Chicken or the Egg” theory, the results were clear.
The Cassidy Study (2008 & 2017): This massive study looked at millions of person-years. They found that patients who visited a chiropractor did have a slightly higher association with stroke. However, so did patients who visited their Primary Care Physician (PCP).
The Findings: The risk of stroke after seeing a chiropractor was identical to the risk after seeing a GP.
The Conclusion: Since a GP visit does not involve neck adjustments, the increased risk in both groups is because patients are seeking care for a stroke that is already in progress, masquerading as neck pain.
The Kosloff Study (2015): This study looked at millions of records from US commercial insurance and Medicare.
The Findings: They found no significant association between chiropractic visits and stroke in the commercial population.
We also look at the biomechanics, which is what physically happens during an adjustment.
Anatomy: Invasive studies have shown that the stretch placed on the vertebral artery during a standard adjustment remains well within the normal, elastic range of the vessel. It does not exceed the “failure strain” required to damage the artery.
Daily Life: Your arteries are designed to move. Activities like backing up your car (common here in Palm Desert parking lots), washing your hair in a salon sink, or playing tennis involve similar head positions. If healthy arteries were fragile enough to tear during an adjustment, we would see epidemics of stroke from yoga, swimming, and painting ceilings.
You might wonder, “Can you test me beforehand to make sure I’m safe?” In the past, some therapists used “provocation tests” (twisting the head and holding it) to check for dizziness. However, research has proven these tests are unreliable.

We look for specific warning signs (often called the 5 Ds):
Dizziness: sudden and severe.
Drop attacks: sudden fainting.
Diplopia: double vision.
Dysarthria: trouble speaking.
Dysphagia: trouble swallowing.
The biggest warning sign: A pain that is “unlike anything you’ve felt before.” If you have a sudden, sharp, severe headache or neck pain that feels distinct from your usual stiffness, tell us immediately.
Your safety is not a game of chance. The highest quality scientific evidence available today (studies covering millions of patients) shows that chiropractic care does not increase the risk of stroke compared to visiting a medical doctor.
